You can call or visit them anytime Mon. & Wed.: 8:00am 7:00pm Tues., Thur. & Fri.: 8:00am 5:00pm. If you are a patient at CHWP, call our office at 937-599-1411 option 3 to speak with a nurse, who will discuss your care with a provider.
If your symptoms are mild, we can either arrange a Televisit or an in-person visit at the Bellefontaine Community Health Center.
